Appointments
All patients need to be registered prior to consultation. You can book an appointment ONLINE or call 7704996699. The registration fee (one time charge) is Rs. 50/. You will be issued a UHID card which is valid for all services of the Jivak Ayurveda. Your medical record folder will be retained with the Jivak Ayurveda.
If you are already registered, kindly mention your Jivak Ayurveda UHID number when taking an appointment.
In case you are a non-appointment/walk-in patient, you may have to wait to see the doctor. You can visit our OPD from Monday – Saturday from 09.00am to 06.00pm and Sunday from 10.00am to 05.00pm.
Consultation Fees
First time consultation charges with doctors:
Specialist doctor’s consultation charges are Rs. 250/- (Rs. 200 for consultation + Rs. 50 for UHID card)
Follow up
Consultation with the same doctor within 10 days is free.
